Position Summary

The Community Health Worker ("CHW") will be responsible for carrying out the assigned initiatives, such as the development and implementation of patient improvement projects and strategies. The Community Health Worker will help develop and implement Population Health initiatives and processes for Darsalud Management projects and the Quality Services program structure.
A CHW must be comfortable interacting with patients (over the phone and in person). They must be able to confidently and respectfully explain the need for healthcare services and the importance of taking care of yourself. CHWs act as patient advocates, and effectively communicate with providers to ensure a patient's needs are taken care of.

Responsibilities

  • Schedule appointments for patients
  • Answer the phone when patients call, send messages to providers if needed
  • Call patients to remind them of their appointments
  • Participate in special projects as assigned, such as the Social Determinants of Health, School Based Health System, Care Coordination, Health Maintenance, Patient Education, etc.
  • Utilize EHR software
  • Must be detail-oriented and highly organized
  • Must have knowledge and understanding of medical practices and organizational policies
  • Must be able to maintain confidentiality at all times
  • Assist in the planning, facilitation, and coordination of population health activities related to the pursuit of continuous improvement in the following: the delivery of patient care, achievement of clinical outcomes, operational efficiency, patient satisfaction, patient safety
  • Perform other job functions as requested or assigned

Experience Requirements

  • Good interpersonal, customer service and active listening skills
  • Ability to manage change and produce results in a fast-paced environment
  • Strong analytical, communication and presentation skills
  • Excellent computer skills and knowledge of Microsoft Office Suite (Word, Excel and PowerPoint)
  • Bilingual (Spanish and English)

Education Requirements

  • High School Diploma or Equivalent
  • Valid Driver’s License
